Apparently the confusion is propagating.

You DO need the NV4500 BELL HOUSING, you DO NOT need the BLOCK ADAPTOR PLATE, which, for some ungodly reason, people keep calling the "Bell Housing".
You'll need the following on the mechanical end of it:
4wd Transmission, with the shifter assembly and handle.
Flywheel, with bolts.*
Clutch and pressure plate, with bolts.
Transmission BELL HOUSING (not "block adaptor plate", your auto one will work).*
Clutch Fork.*
Hydraulics, Slave and Master.*
Clutch pedals/assembly.*
The parts with '*' are pieces I have here.
Your current starter is the same, if you use an NV4500.
Some 6 speeds use a different flywheel with a revised ring gear location, and therefore use a starter and a spacer that's unique.
As I mentioned earlier, I have most of the set-up to convert you to 5 speed and you can get rebuilt 4WD NV4500s off Ebay for @ $1300, with a warranty.
Also, keep in mind, when you swap to the 5 or 6 speed, the transfer case in your truck now is not compatible with the manual transmission.
An AUTOMATIC uses a 23 spline output shaft, while a MANUAL uses a 29 spline output.
This necessitates AT LEAST swapping the transfer case input gear, or the whole transfer case.
