Scan for codes. Could be VP-44 could be APPS - could be a few other things.

What is the condition of your lift pump? do you have a fuel pressure guage?

You need a fuel pressure gauge to check the lift pump.
P0121 (M) Accelerator Position Sensor (APPS)
Signal Voltage Too Low
APPS voltage input below the minimum acceptable
voltage.

You might be in luck, and it could be just the APPS. You can get an aftermarket one from here:

What he said. ^^^

He needs to seriously think about putting a fp guage in. Can literally save him over $1k.

You can request a price quote from the vendors here
Price Quote - Request A Price - Diesel Bombers
You can expect to pay around $400 for a three gauge setup though I think...
An aftermarket lift pump is an absolute must on a VP powered truck (FASS or Airdog).
Good luck.
