You will not get 25mpg in these trucks... They are big, heavy and have the aerodynamics of a damn brick. That being said, I have gotten 20mpg one time. Highway driving keeping your speeds around 60-65 and you will get yourself close to that.

Now with that truck having 250k miles... It might be an issue with injectors that is causing this. Does your truck idle nicely or is it kind of rough?
No disrespect. there are two little filters on the the sending unit pic-up tube in the fuel tank. Best way to test is to hook a fuel gauge in the fuel bowl and see if your loosing pressure in Certain rpms. If you get the pressure but it drops and the truck stutters I would check in the tank. Its cheaper to eliminate that problem. If it the pressures are fine, then your good from the tank to engine. Your next test is possibly the injectors.
I still think you might want to look into the Cam position sensor. I had the same intermittent stuttering.
good luck.

This happened in the cascade region last year going Lassen park. We have some mountains. At first I thought it was the fuel pump? It tested fine. It took dropping the tank to figure out the issue. I am not saying this could be your problem. Every truck does something different. All we can do is send you on the right direction to get you to diagnose the problem. None of us can really effectively diagnose a truck unseen. But don't quit asking questions! We can still give you ideas on where to look next. And we like to read how people are able to fix their truck problems.

Just to let you know 24mpg highway can be obtained. I am getting 16.5 mpg city as we speak. I don't know what I am getting on the highway yet, and might not be able to find out!
