You should be able to have a driveshaft shop make you a custom unit. Or they might be able to suggest where to find a large yoke. You can also check the internet for shops that specialize in rock crawlers. They usually have a good line on custom yokes and such.

If standard, do an NV4500, it'll bolt right up you'll just have to get the driveshaft stuff done.
Auto...... maybe a 47RH or comparable Dodge tranny? Not sure on that one.
