Default 6.0 question

Our 6.0 is acting up, it vibrates in idle and feels like it has hesitation when driving. If he pushes it it clears up for a little while and starts again. Also, sounds like it is missing. He said it backfired the other day. What could cause this? We replaced oil cooler, egr cooler, and cleaned exhaust pipes. We also checked egr valve, it isn't sticking open.doubledhotshottrucking@yahoo.com

Oh yeah, it doesn't show any codes.

Going to need a lot more info about the truck. How many miles? Any mods? What is the truck used for mostly? Did this start out of the blue, or has it been getting progressively worse? When you first fire it up, does it take a while before it starts? Does it smoke? What color smoke?

Any more info you can provide will help.

What did you scan it with for codes, or are you just saying it has no codes because there isn't a check engine light?

It is used for a hot shot truck, has 163,376 miles. Started acting like this slowly getting worse now. Cranks right up, but as it warms up it developes engine lope. It has a small amount of black smoke when you push on gas. He used an OBDII.
