I would clear the codes and see what returns as a hard code.
As stated a PMD or a PMD missing the resistor can cause troubles but the situation you describe sounds a lot like a failing IP.
Start with another known good remote mount PMD with resistor,heatsink and 6' extension harness and see if any improvement-all electronic IP owners should have a spre PMD on hand anyway so it's not wasted money.
Also be prepared for a new/rebuilt IP.
