Default Blown OPS within a few weeks

Hello. As some of you may know by my other thread, i was having some problems with fuel. Replaced OPS and it worked like a dream for the past two weeks. Today though, im pretty sure the OPS went out again, as i can no longer hear the lift pump kick on during the Wait to Start cycle like it was during the two week period. I plan on exchanging the current one for a new one from advance auto parts, and also doing the OPS relay mod. The only thing im wondering, is if there is anything else i should look for that would cause the OPS to go out this soon? Its not like i have been beating on it, just driving it as a 'normal' person would to work and back. I also ask you all for you opinion, as to the best way to do the relay mod. I have seen it a few different ways, with the main variance being the power to the added relay. Some people splice into the OPS's 12v volt wire, will others add a new 12v wire from the firewall block.

Thanks in advance

It was AcDelco. Would not order anything different. After reading a bit more, im even more confused. One OBD-II trucks, the fuel pump relay gives power to the lift pump during the wait to start. The past two weeks i was able to hear it kick on, and now i can. So faulty fuel pump relay you think?

Went and picked up a new relay today. While doing so, i tried jumping the lift pump as if i were bleeding the system. The pump did not kick on. So now off to fight one of my biggest fears, wiring. Im in class till 9 tonight, but dont have to work until 11 tomorrow, so ill get out the test light and see what i can discover.

I did replace the lift pump when i replaced the OPS, so both are not even a month old.
