Not likely, if there is failure in an injector, it would produce lots of smoke as the fuel will not burn properly. Could there be valve train issues in the #4 cylinder area? If you are up to it, swap injectors from cylinder to cylinder. Maybe move #4 to #1, and see if the noise follows the injector, or stays with the cylinder.

I would assume if you had a worn guide, you would have oil consumption as well. To my knowledge, the best way to test a valve guide is to remove the valve from the head, and use a inside bore gauge. There are special guide measurement tools as well. But this requires the removal of the head. You could remove the spring from the valve, and give it a very rough "feel test". If the piston is at TDC, it will not drop into the cylinder. I am not sure what kind of tool you would use to relieve spring tension to remove the keepers. All the tools I have seen are used with the head OFF the engine. Have you tried cracking the delivery lines while running to see if the noise changes? Try #4, and maybe a few others as well, and compare the sound differences. Of course, take precaution with this, high PSI.
