Default Transmission advice

Hi my name is brandan I have a 1991 dodge w250 12v and I'm not really familiar with transmissions and last two times I've drove my truck it shifts fine for a while then it will start to feel like the transmission is not engaging all the way it just started do this I've changed filter and oil all fluid levels are full I just don't get why is it going out.
