Question engine skip????

the engine on our 987 f250 has 91,000 and it also has a really noticeable skip in it when it is ideling, and when ur running it, when ideling it is like a soft punch on the sholder, only iI can feel it throught my feet. Did you guys have this problem and what might cause it (I'm new to powerstrokes). No water in fuel, new fuel filter, air filter is new????? (I'm not used to electronic engines I like my old mechanical IDI's).
