Explain why some vehicles have a charge air cooler and others don't?

It mostly comes down to whether or not the vehicle needs it vs. how much the manufacturer's bean counters feel they need to spend on vehicle production.

well newer diesels are intercooled, no? I was wondering if there was some specific reason mine wasn't, and what benefits i'd have if i put on an intercooler setup.

The OBS were not intercooled... None of the offerings in those years were. IIRC the 6.5 was intercooled in 96, but I cant confirm that.

Stock injectors... No real gain by intercooling it. Chip and maybe some stage 1's... Its nice. Above stage 1's then an intercooler becomes a nice add for towing.
