cold start problem
the truck is an 04 f350 with about 160.000 on it. it has an egr delete and straight pipe and a superchip. my mom has used it to pull a horse trailer all its life and to and from work. well in the morning when she starts the truck is very ruff, and then it will try to stall and than catch it self and blow some black smoke and than idle real ruff for about 5 more mins and than run perfect after it warms up. it warms up for about 15 mins every morning ( in Florida ). what could cause this thanks

A lot of things. Going to need some more info. Any check engine light? Even though there is no light, try reading the codes anyways, sometimes they throw a "soft" code where it wont trip the CEL. Has any of the major stuff ever been replaced, like injectors or turbo or anything? I would try and get a cylinder contribution test and injector buzz test done.

The first two, p0403, and p0405 are EGR faults, obviously because your's is deleted. Only way to get rid of those is to get an SCT with custom tunes.
P0460 is a miscommunication between fuel sender and gauge cluster, here's more info 06-5-10***MIL ON WITH DTC P0460 - 6.0L
P0528 is Fan Speed Sensor Circuit No Signal, most of the time it's just the fan clutch that has failed. You can check the wiring for this under the intake elbow on top of the motor.
P0678 is #8 glow plug. This could be part of your starting issues....
p1000 is nothing, just means it didn't do a full cycle to complete testing, not an issue and the code will clear itself after some driving
P2285 is ICP (Injector control pressure) sensor, and this is most likely what is actually causing the hard starts. I believe on 04's it's on the back of the motor behind the turbo and a royal PITA to get to. Oil often would drip down on to the wires and cause the sensor to fail. A lot of times the actual harness that plugs into the ICP is burnt up too. I've heard of some guys cutting it back to where the wiring was good, putting a new sensor in and being good to go. Supposedly there was enough extra wire back there, but don't count on that. That is where I would start looking. A lot of times when the ICP fails, it will leave the truck stranded, so if you can actually get it to start and run, consider yourself lucky (in some twisted way).
Hope some of this helps.
