verify tht glow plug module connections are pressed in firmly and all the way if even slightly not quite in all the way it will the above but I would also look more into the fuel regulator if faulty and not taking care of itll wipe them injectors out clean

 

also verify ground wire for the ficm is grounded it goes on the rearmost drivers side intake manifold stud
