Default DMAX Blowing Off Intercooler Lines

Have a 2005 Duramax that has three times now has blown off the intercooler hoses twice at the top drivers side which was siliconed by the dealer and now on the opposite end of the same hard line just before the intercooler. Anyone else had this happen? Truck has been kinda weird now with chimes going off after the keys are removed when getting out, same chime as headlights left on. Has it lost it's mind? Don't have any warning messages or idiot lights on.
It's close to being out of warranty, should it see the dealer?

Thanks.

All of the above tips are good advice.

Also. make sure that your turbo isn't passing oil out the compressor discharge. I've seen a number of the LLY turbos do this. The oil being passed makes it easier for the boots to slip off.
