Default clutch issue

ok when i did my ford cummins conversion i put my 94 cummins in a 91 f350. ford cummins told me to use the ford master cylaner and clutch line with the nv4500 slave cylandar which is what i did. took it on a trip last weekend the clutch has to go all the way to the floor to disengage. when letting the clutch back out maybe 3/8" the clutch grabs. and after 2 days the master cylander would be empty. any idea?
