220 is hot but not hot enough to be worried but if it would be around 230 or more then I'd be worried, but fan should kick in full bore and cool down. But 240 and over then you got a problem.
But with what your pulling though, I would expect it to be cooler. But you do have a older truck, but has the radiator been clean on the outside for mud or debris? Might help to maintain the heat and keep the temps lower.

one year ago november i installed a new radiator, new water pump, new thermostat, new fan clutch,,,,,,,BUT... the radiator was a 3 CORE HIGH EFFICIENCY RADIATOR. it had a 4 CORE radiator in it, 10 years old and it sure enough wasn't doing the job.. the radiator shop owner assured me the 3 core high efficience would do the job. what do you think about that 3 core????? thanks r young

A 3 core high efficiency is pretty much equivalent if not better then to a regular 4 core.
Have you changed the rad cap, or tested it? If cant hold pressure, it ll heat up quicker.
Do you know your intake temp and egt's when your at 220?
