flush your cooling system. literally stick a hose in it and watch the **** blow out. clean your radiator. make sure your fan clutch is working properly.

those were the biggest things for me.

The radiator is 12 years old and probably scaled up. Aluminum radiators have a few good years then its time to replace them.

you need to clean out your rad. its a must. also make sure thermostats and fan clutch are engaging at right temps. dont let your truck go over 210 for very long.
