Default bad hydraulic throwout bearing?

'97 Chevy 4 door 3500 4x4, 6.5 diesel, 163,000 miles, 5 speed,
Replaced clutch hydraulic master cylinder, bled several times, seemed to be straight fluid.
To back up a little, went out to move truck and would not go in gear. Changed the master cylinder and went in gear fine the first ten times or so. Thought everything was good. Went for reverse, no way, would not go. Shut off truck, put in reverse, hold in clutch, all good. The more you drive, the worse all gears get. Have heard that the ballstud and throwout arm wear out and could cause problems. Have new hydraulic throwout bearing in box. Should I be replacing throwout bearing or ballstud and throwout arm? I realize once I tear this apart, this may be self explanatory, I think, but would like some input from someone that has been through this. Truck sits alot, but need it to work when necessary.
