Do a LP and OPS test,Put clear hose on the return line on the IP to see if you sucking air

Also recheck the fuel filter to make sure it seated right

I'm going to go with a guess. You have a pinhole sized leak between the IP and the fuel manager. The hose has been in an unbelievably hot area for 16 years and is likely rotting and slowly allowing some air in. The fuel manager has a line into it from the back that can also become brittle and weak/leaky.

You could also have a weak lift pump.

I have a 99 gm diesel and mine is doin the same thing. i put it on a copmputer and it said the cam position sensor is junk. The problem is I was told the cam sensor is built in the injection pump so now I have to replace the whole pump. $1,000 down the tubes!!!

Ive been doing alot of research 0n the 94 6.5, the pmd causes alot of stalling problems, the best thing to do is relocate the pmd away from the engine so it wont over heat, dealerships will tell you need a injector pump, somtimes its just the pmd it take bout 20 min to do ,heath diesel have the best product with 7 year warranty
