have you figured it out yet?
If not I had a similar issue that turned into a no start... I had no smoke at first and it ended up being a bad injector. I would start by checking my battery for a full charge, cleaning all sensors (V.C. gasket harness, map, its, ckp, and csp sensors with electrical connector cleaner) and then do a lift pump flow test.

Chase your fuel down. Got full filter housing? (if not possible lift pump) Got solid rail pressure? (if weak or all over the place cp3 pump) If these are good, could be injector. Lift pump on early 04 (305 hp) is behind the filter but there was and update some dealers done and sometime when they changed to 325 hp they put them in the tank.
Also is there a check valve on the return side on the CR? I know on some large trucks do this but not sure on the 5.9
