I've got it done. This is on a Dana 50 HD...
Pull the wheels.
Pull the calipers.
Open up the hubs and pull the guts out and snap ring.
Use special spanner socket to pull out wheel retaining nut.
Pull out the PITA lock washer.
Use special spanner socket to pull out bearing preload nut.
Pull hub and rotor assembly.
Have your 2 beater nuts ready...
Thread them on and beat out all the studs.
Put new rotor on end of hub.
Drift studs in from behind the rotor into the hub.
Torque down your beater nuts on all of the studs to clamp the hub and rotor together.
Repack the bearings in the hub.
Installation is reverse...
Put the hub and rotor back on there.
Use spanner wrench and preload the bearing to 50lb-ft while turning the rotor.
Back the preload nut off and retorque to 40lb-ft and back it off 90 degrees.
Install PITA lock washer and it must integrate into the axle AND the preload bearing. Good friggin' luck - about drove me nuts.
Once you get that install the retaining nut using spanner socket and torque to 175lb-ft.
Reinstall snap ring, hub guts and hub.
Reinstall caliper using new pins.
Reinstall wheels.
That's from memory. Might've missed something. Good luck with those sh!*@y friggin' lock nuts. Just dealing with those alone made me want to drink.

Lining up that spanner nut with the ring with the pin is a PITA. I was unable to separate my hub and rotor, all I did was trash a few stud threads. Luckily they guys at Les schwab have know me for years and separated and reassembled them for me as a favor.
