Default 2003 7.3 lack of power

I have a 03 7.3 PSD auto. trans FX4 with 289k on it, it is not performing as good as a 7.3 should, this is my 4th 7.3 and my brother has 2 I know my way around them a little but im kina stumped here. The truck starts and idles great almost no blow by typical oil seepage for the milage I just got the truck 2 weeks ago, got rid of that pos 6.0, and went back to something I know a little better, but any ways it starts and idles great and from 0-25 runs ok but after that it falls on its face checked hpo and its building 2500psi 20lbs of boost fresh oil, oil filter, and fuel filter the only codes are p1211 (icp pres. not controllable above desired) and p1209 (int. control kam mod err.) which are typical with a programmer it has a new icp and cps (ive put the old black one back in place of the grey, which made no difference) what am I missing, where else should I look, what should I try, any help would be greatly appreciated thank you.

You said new oil, oil filter and fuel filter but how's the air filter??? I had a 99 that the stock cheap air filter looked ok and it had the filter minder pegged. Drove like it had nothing after 1st gear and had to feathered to keep it running at high rpm. Anyways something to check before replacing ICP sensor.

You said it idles okay? Then going down the road you loose power. Does it run funny after you run it and it falls on its face? Because I've said once before the Cam shaft sensor went out on mine and it would run for as long as it wanted too. Then it'd fall on its face still idle but idle like it was about to explode. I then took it to Ford and they jokingly said it needed long blocked (say jokingly because this was wrong). I then pulled the tank thinking there was some trash in the tank or bad fuel. I then cleaned the tank put fresh fuel in it and it ran for probably 5 miles then started doin it again. Let it set in the shop for a month. Then someone came buy and said it was the Cam shaft...changed the came shaft sensor and shes been runnin okay..not back to normal but thats because of another issue that I'm soon going to fix. Thanks to some vacum lines. LOL...let me know what you think about that..and if you've already done that...sorry donno all the code names for pick ups since I mechaniced on my own stuff and was short lived in the pick up shop that I worked in since they went belly up.
