Default P0265 Code, 2003 6.0, Truck bucking and no power

Hey Guys,
I am new to this forum. I have a 2003, F350 6.0L, with 220,000kms (mostly summer use) on the clock. I have owned the truck since new. I had injectors replaced on one side (not sure which side right now) at 100,000kms due to a fault in one injector. I have not had any issues since then until recently. About 3 weeks ago, I replaced the alternator due to a low voltage issue and, at the time, repaired a wiring fault. Prior to this, the truck ran great except, for the past 5000kms, every now and then (seemed like when it was wet - possible coincidence) the truck would intermittently miss and a couple of times the truck shut down while driving but always restarted right away. Battery light would intermittently come on and figured the problem was related to the alternator. After the alternator replacement and wiring fix adjacent to the alternator, over the next 2 weeks (1600kms), the truck ran flawlessly. Then last Friday, the truck started bucking, lost power and started "puffing" black smoke while towing 10,000lbs. Now, same thing occurs while running empty. I changed out the fuel filters, which did not seem to remedy anything. There were not any codes.
As of Tuesday I have a P0265 code - Cylinder #2 Injector circuit high. Any help would be appreciated.
Thanks.

Found my problem. It was a chafed wire to Injector number 2. There is a little section of wiring that runs from the main harness to the injector that does not have any loom on it. One of the four wires chafed through the coating on a boat head on top of the engine and was arcing on the bolt. some shrink tubing, electric tape and a small piece of wire loom and all good again!!
