Default HELP

My 92 d250 is running like crap. I changed the filter and fuel pump and it still wont run right. It acts like i am barely mashing the gas and is smoking gray smoke like a freight train. I looked after i had cracked the injectors to try and force out any air and my belt has poped off. Does anyone know what my problem may be and the belt pattern so i can put it back on. Thanks
