06 Duramax Shaking at an Idle

06 Duramax Shaking at an Idle
i have an 06 GMC 3500 Cab and chassis with a 6.6L Duramax.
It is shaking really bad at an idle and smooths out a little at higher RPMs, When first looking at the motor i was thinking maybe injectors. After doing a cut out test it seems like all injectors are firing properly.
I did find the balancer bolt was backed all the way out.
A buddy seems to think this is a balance issue and not a fuel problem
I need HELP!!! This is an over the road truck!!!
PS after tearing down to the balancer it appears that the balancer is in good shape, is it possible that it could have spun on the rubber or could the key-way have sheared?

I think I know of a guy who is selling an lbs balance actually.
Empire diesel performance sells a kit to pin the crank shaft. Check out their site, I know there are other companies who offer something, maybe fleece performance
