there can be depending on shape and model. if its been treated right you shouldnt have too much issues. glow plugs and injectors are lifed items and like to go if they are old. injection pump and lift pump can crap out on you. and on newer versions(97-98) blocks can crack and heads can crack. if you dont drive it like a cummins you will be fine.

head gasket can be problem but if fix it right it last you when you replace head gasket throw the bolts away and go with ARP studs and with that few mods it be good truck

Everyone I ever had I ran the crap out of and they did just fine. There are some problems with the 6.5 but most of them have been blown up to JAWS proportions. In reality if you take half way decent care of them and do some mods they will be fine. If you get the truck when you replace the head gasket make sure the deck and head surface are flat and not scored. Ignore the cracks between the valves it is just a surface crack and will not leak water and put head studs in it or in the vary least replace the head bolts.
