Default DTC Codes

I have a 2003 3500 with an auto, about a month ago my truck lost power and developed a rough idle. I checked the codes with the Smarty and it read P0204 and P2149. I googled the P0204 code and it was for the #4 injector. I replaced the injector and the connector tube. The truck started up and ran fine for a week. Then I was going out of town for a weekend and the truck lost power again. I checked it with the smarty and it was the P2149 again. I had the truck towed home and it started up just fine. Drove it again and the truck lost power. Again it was P2149. Cleared the code and it drove fine. Now my truck is parked. Any help would be greatly appreciated.
Thanks
Mike
