Default 03 Cummins 5.9l H/O lopes when cold?

Anybody know why this happens? Most of the time it does not lope when its over 60 degrees outside, otherwise it sounds like the engine is missing when i listen to the exhaust and the rpms surge up 850 and runs like that until it idles for a few minutes. But it also only happens when its out of gear, it fixes itself when its in reverse or drive. This has mind boggled me because there is no smoke from the tail pipe either and it starts right up. Anybody have any thoughts on what the issue could be? bad timing? Sensor? injector(s)? valve lash? im stumped

If anyone ever revives this thread or even replies to it, then well the fca was not the issue. Took the valve cover off to get to the injectors and a connecting tube was loose along with some of the injectors sticking and not spraying properly! be warned when your diesel lopes when its not suppose to!
