The reason for the 2 cycle is to lubricate the VP-44 injector pump. This injector pump is cooled and lubricated by the fuel. With todays ULSD, most feel that there is not enough lubricating properties left in the fuel by the time it makes it to the diesel pump. And yes, you have to add it every time you fill up.

there are a number of people who do the same thing with duramaxes, i did it once, it made my engine seem sluggish, but after that my mpg jumped up and my truck felt new again
