You have a blown oil cooler. Your oil cooler was stopped up limiting flow and now has ruptured. This elevated the coolant temp which boiled/ puked causing the head gasket failure. This is why I and a lot of others reccomend a new oil cooler and either a new egr cooler or egr delete when you do head gaskets. Do your self a favor and get a new hpop screen from ford. The new stainless screens are improved. Also when you replace the oil cooler and the egr get a coolant filter to try to reduce the chance of having the oil cooler plug up again. If you want to stay on top of things get a set of gauges either digital or analog that will tell you coolant temps and oil temps. They should be 15* or less differential in temps after the truck is at operating temps
