bad injectors or bad injector orings. Just too much fuel entering cylinder. Be cautious though because when 6.0's over fuel too long they can hydrolock and bend connecting rods, score and melt pistons outta them too. I had one come through my shop that seized a piston to cylinder wall, the rod and wrist pin pulled out of the bottom of the piston and hammerd the bock till it was junk. Customer said he was driving the pickup and it was getting a little "weak" then it died while driving. Once I pulled it apart I was like...." yea, im sure that's exactly the way it went.... NOT " haha So be careful
