Confused with the posibilities!

Hey Guys!
I am stumped on this one. I would like y'all to help me narrow this one down a bit.
Here's my issue:
On my return trip from SC, I noticed that I am losing power while driving.
It goes like this:
1. From a dead stop, it seems to accelerate pretty good at about 50% throttle. Any more throttle than that and it makes absolutely no difference.
2. when cruising speed is achieved, if I go to pass someone or add more throttle to climb a grade, it seems like I almost have to mash the peddal all the way down! It downshifts fine, but very little additional power. Throws a little more smoke than usual but not too much. I can hear the turbo spool up ok with around 3-5% more throttle. (so I don't think it is the VGT)
3. fuel pressure and oil pressure good
4. cranks fine- no issues there
So, I stopped at rest area and pulled the dtcs. It threw P341 (camshaft sensor).
Here is what I have done so far to correct the issue:
1. Checked all CAC tubes, boots and connections
2. Obviously replaced the Camshaft sensor. - made no difference
3. Changed both fuel filters and drained water seperator- Made no difference
4. Went ahead and changed oil and filter while i was there.
I know there is an extensive list of possibilities. Here are a few of the ones i have come up with:
EBP sensor and line, tps, map sensor, potentiometer.
What do y'all think?
Anyone out there run across this same issue?
please list you thoughts in order of "most likely".
:humm:

Did this just happen out of the blue? You said oil and fuel pressure is good right? What is your fuel pressure reading. A stock tuned 6.0 really shouldn't have ANY smoke. Being you have an 03, you don't have a boost gauge, but do you monitor boost at all? Me thinks the turbo just may not be allowing the vanes to open all the way.
