2004 Powerstroke acting up

I have a 2004 f250 powerstroke and it recently started running funny again. This truck was bought with 20k miles on it from an elderly couple who didn't use it for anything other then driving 500 miles to their winter home in the south. since i've owned this truck for several years now and put 45k miles on it I have had quite a bit of problems already with it. I've went through 2 EGRs and a transmission module of some sort. the EGR was just replaced recently and lately it has been surging while idling and once when i was cruising down the road around 55mph at 1600 rpm or so. Once you turn the truck over from cold it will take about 15 miles of highway driving before you can get it to act up, and its when you slow down to 5mph or less it will want to die or surge and get real jumpy. When the trucks been moving all day and you let it idle for more then a couple mintues it will really act up then just idling with pretty severe jumps in rpm. i noticed a smoke come from the tail pipe but couldnt really determine weither it was black or blue i was standing to far away. i just recently changed both fuel filters and oil, but the problem was occuring before that even. I tried putting an a diesel additive to help clean and lubricate the fuel system but it doesnt seem to have helped. i really hope this information is good enough for someone to help me out and at least give me an idea before the piece of fine shit has to go back to the dealer for another couple grand in repairs. we don't use the truck for much all it really ever pulled was a double axle trailer with a skid loader. :argh::argh::argh:

I love the 6.0

Not a real good description but from what im hearing it sounds like it might be the egr either the valve or the cooler. But it could be a bad injector as well without scanning it you could spend alot of money throwing parts at it.....An easy way to tell if its the egr valve is to disconnect it at the valve if your surge goes away then thats where id start...But if the valves bad you should have a check engine light..A friend of mines pickup was doing the same and he had a bad injector....Bottom line is scan it and find your problem...
