Question trans or torque converter?

the other day i was drivin my truck and it seemed like when it went to shift it would jump up about 500 RPM's before it shifted into the next gear...once in gear it drove ok...i was driving it the other night and comin up a steep hill and i started hearin a thumpin noise and viberation through the floorboard....now between 1500 and 2500 it likes to rev up and looses power.....what do yall think?
