Default strange ticking noise at back of engine

when the truck is fully warmed up and only in either PARK or NEUTRAL when the idle is up higher i have a trucking noise coming from the back of of the engine and when i hit the gas pedal it does not gradually increase as a valve tick would so i know it isnt a valve and it will NOT do it in drive reverse 2 or 1 gear. any one know what this it? it only started when the truck was ran out of fuel and has to crack injectors to bleed the air out. truck has 200,058 miles as it sits and still runs very strong. any help appreciated
