Difference between cummins engines?
The difference is 45 horsepower 
The pumps were all P pumps but the autos got a slightly detuned version. I think the only difference was the delivery valves but I can't remember for sure. They may have had a slightly different plate between the two also.

I am confused here - the most stock hp Ive seen from a 12V is 215 hp from a 96-98 standard shift truck.
What years are these trucks you are talking about?
Are you talking about factory installed pickup applications, or are they other medium duty Cummins engines?

I am talking about a BlueBird bus. In the original production order form theyve had an 12V 5.9 TA 190HP with AT545 trans as standard.
The ordered one was a 12V 5.9 230 HP with an MT643 auto trans...
The pickup date was the 28 of August 1998.
The title about the bus says its build in 99. So I guess it was one of the last 12V.

their are many variables, but in the end its all basically the same chit, different pile.
Their may be a few differences in the pump (cam,DV's,calibration, timing), it may have slightly larger injectors
Your talking about a bus engine so forget the whole 98.5 switch to 24v, the 12v was still avaliable long after dodge switched, we were still buying new 12v's up until a few years back. As for the 'standard version' HP, their is no standard version, cummins builds the engines to whatever spec is required, they can be bought 'off the shelf' in various power levels or calibrated to whatever suits the needs of whoever needs them.
Then if you look at a marine engine they are the same bird, with different pistons/injectors (different spray pattern), and also rear geartrain and counterotating in some applications.
