idk if there is a place on the internet but if you have a local driveshaft place you shoud be able to go to them and get a quote i would say its going to be in the 700 to 800 dollar range to get one built..

X2... Any local drive shaft shop, heavy duty truck place can make em.

Anyone running a one piece?Any advantages besides the the fact of no more changing carrier's? Just curious.

You need to be careful swapping to a 1 piece driveshaft. The stretch you need to span may be to far for a single piece ds. In most applications they came with a 2 piece shaft for a reason. As far as your carrier bearing, you want Dana-Spicer part# 211848-1x, thats the OEM bearing, and if you pay more than $65 for the bearing your getting ripped off.
