Injector hold downs snapped in half?!

About 1000 miles ago had the head off and all that good stuff. I bought new 125hp injectors and installed them when I put everything together. The truck ran fine till last Thursday, left the gf's house and got on it a little bit and it sucked air and shut the truck off. I cracked the lines and got the truck running again but then the fuel tank started pressurizing big time so I took the cap off and limped it home. The next day I popped the valve cover off and cylinders 1&4 injector hold downs were cracked in half!!!

What would the culprit be guys? I assumed a injector or crossover tube didn't seat and was putting cylinder pressure into the fuel return line making the tank pressurized. But the injector hold downs have me at a loss... When I torqued them I made sure they were at 89 inch pounds.

A shot in the dark that I have though of;

Sealing washer(s) failed, allowing combustion pressure to come past the nozzle tip. Now that pressure can act on much more surface area of the injector, pushing up way harder than when the pressure only acts on the nozzle tip. If combustion pressure did get past the nozzle, it may have gotten into the fuel return system and made the truck shut off.

A shot in the dark, but it's all I have right now.

well yesterday I got it back up and running, when I pulled the injectors you could tell that 1&4 had turned black from the cylinder pressure getting in there. Replaced the caps and O-rings, coppers, and the delivery tube O-rings as well and she seems to be running fine (for now at least)...
