Interesting thread.... and issue.
Have you considered maybe getting a mechanic's stethoscope and probing around in an attempt to "pinpoint" that noise? Just tossing it out there....
There's gotta be a logical explanation...….

Never had luck with a stethoscope on a 12 valve .

Indeed. Mine doesn't sound like that on a cold start, but that could be because you didn't exactly give it any help in that video. Dunno if you did it that way to reinforce a point or if that's how you always cold start it, but either way- years back we were talking about the sound of exhaust from different brand diesels and someone commented that to them a Cummins sounds like a moose farting in the wind. It wasn't until this topic that I had any idea WTF he meant by it. Regardless, I wonder if it would still do that if you start it according to the owner's manual. As for the TC, I believe his issue doesn't go away with warm up, and only lessens as RPM rise but doesn't go away. So it's possible that his idle is set too low, but it sounds like that is only exacerbating the situation not causing it. .

I honestly think its your idle, get a Tach kit for a 12 valve, they arent expensive.

Thanks a lot again guys for all the replies and the help. I just purchased a hand held tach and if this thing is reading correctly I’m idling at 700rpm with the A/C off. I’m going to bump it up to 900rpm and see how it sounds. I will let you all know how it goes and if it at all remedy’s the moose fart 🤣🤣

Just another quick thought. Might be a dumb question but I’ve always been told no question is a dumb question so would a bad overflow valve not producing the right fuel pressure causing the injectors not to spray enough fuel or atomize correctly cause such an issue? To me it sounds like it could not be firing 100% causing the flapping noise. Truck has 250k miles on it and I have to assume it has never been replaced.

Well boys, all that happened by bumping up the idle is that it just makes the flutter faster.... I dont know what move to make from here unless someone says the overflow valve could be it but I highly doubt that’s the case.
