You got me curious. I am on my second gear in my Mega. I think it would be nice to add one of these and maybe help this box last a little longer.
Only thing I know to do is make sure you don't steer without the tires rolling. If you have ever been towed with a dead engine then you know how much more force it takes to steer sitting still as compared to rolling!

With bigger than stock tires I would consider the steering brace a must have. This has been a problem in the Dodge Cummins trucks all the way back to the 1st Gens. They tended to crack the mounting plates and frame behind the boxes. The newer trucks are better designed so they just hoof the gear boxes themselves. Combination of the Cummins weight + any added stress like big tires, off-roading, etc. spells an early death to the Dodge steering gear.

if you have a 1st gen and the steering bx bracket is already broke can you just weld it or does somebody make an aftermarket piece that gets rid of the problem

If you're talking about the mounting plate that goes between the box and frame. you can get them through a Dodge dealer. They were still available a couple years ago when I got one. It was like $23, IIRC. If not, you will just have to repair it as best you can.

Whatever you do, make sure to beef up and brace the frame behind the steering box to prevent reocurrence. Since mine was not damaged yet I used a new mounting bracket to make an inner box section for inside the frame behind the steering box, gusseted that down to the crossmember and reinstalled the box with new grade-8 20-pitch bolts:

Can this be accomplished with just a cross member between the frame rails or does it have to incorporate the steering gear to work.
