Generall causes for white smoke on a CTD is bad fuel filter, two weak lift pump, stiction in injectors, or injection pump. I doubt its the injector or the injection pump because it only at load not neutral, so Id check fuel pressure and your fuel filter.

A pump that's just beginning to fail (timing failure) will first produce issues under load only and progress into at all times in many cases.

I have a bad sending unit in the tank, could that cause fuel pressure issues? My new one will be put in Tuesday.
