sounds like your MAP sensor is shot
voltage too low, voltage too high too long, overboost, etc...
sensor is throwing too low and too high of voltage readings that are out of spec which usually means either a rodent chewed through the map wires, or the map sensor itself is dirty/shot and needs replaced...
the sensor is located on the drivers side of the head in about the middle of the head around the fuel filter area.

probably not..
you can take it to a dealer and have them do an injector kill test to see which injector is bad...
if it really is an injector bad then replace the whole set...they are cheap enough to do all of them on a 24v.
common rail would be a different story since their injectors are ridiculously expensive, but lucky for us, we don't have common rails
