I have a 91 cummins with an automatic would it be worth the hastle of changing to a five speed

Well i have had problems with my truck getting hot and my truck is intercooled just when iam pulling a heavy load

i dont think putting a 5 speed behind it is going to help the cooling system unless it will go to a lower rpm. is this the same truck you just posted for sale?
